I have a third intertel 770.4500 IP phone that I would like to enable but I'm not sure how to associate the mac address of the phone into the IPRC card. I created an extension with the DB studio software but not sure how to proceed from here.

What version? It makes a big difference on how you program it. Newer versions are programmed in the Axxess DB where as older versions require that you program the Axxess DB and the IPRC.

Thanks for posting back. I made progress. I got the phone to work to the point where I can make calls to existing extensions in our office, but when I try and place an outgoing call to a land line I get the message access denied.
I program my system using session manager version 5.352 and the db studio is @ 1.36. Any ideas on why I can't make outgoing calls to a land line on this IP phone but I can on one of my other ip phones.

You need to check the CO trunk groups as to what lists are included in Outgoing access to make sure your new extension is included.
