We have an IP Office 500v2(9.1) that was first installed in May of 2015. In August the system stopped working and our vendor replaced the complete system including the SD card. It worked fine for the next few months then yesterday it stopped working again. The vendor worked for 4 hours reseating the modules and trying to reformat the SD card but nothing worked. The vendor is in the process of replacing the whole system again. Has anyone experienced anything like this?

Very strange, I've never had a IP office go bad. There rock solid.
Look for something else, like power or grounding, or bad cabling.

Follow up. I assume your vendor is a Avaya BP, he should be able to get support on that issue.
Replacing the whole system 3 times is not the answer.

Have they indicated what the actual issue is? Is the SD card losing programming, or the IP Office? Early releases of 9.1 had issue where the IPO would not see the SD card, and you would need to rebuild and recreate the card.
9.1.5 fixes this
