Have an IPSLA and when I come off hook I get busy tone. When I call the ext. I get that it is unplugged. My online and IP staus LED's are on. Any ideas.
Thanks in advance!

Does the IPSLA show in you IP device status? Can you ping it? Sounds like the leds are being less than honest with you.

IP-SLA's don't generate their own tones, so I'm curious why you see it as unplugged. Perhaps it's a different extension? Sounds like it's a house phone to a dead trunk group or it rings down to an unplugged extension.

Did this IPSLA ever work or is it new? 5000 or Axxess? If 5000 do you have C licenses and is the IPSLA the correct version? If Axxess, what version and is it connecting to an IPC or IPRC? What version is the card?
