I've got a copy here I could send you but you can easily pick the document up off google as well as the v2.11 software, its an easy procedure I've just upgraded a couple of cards to 2.11, you'll need to make up a serial SIO cable to do the upgrade

The sio cable is the same as the svmi20e (aka cisco console cable).

You don't need the sio cable, just a tftp server (3cdaemon works the best).
If on site i would connect direct to the oas from my laptop via cross over cable (probably don't need cross over, it was just a long cable i had in my laptop bag).
I'll dig up the procedure and send it to you.
Why 2.11 in particular? There are newer versions than that out
