Does anyone know if the intertel phone system automatically disables ports if it detects a wiring fault or disconnect?

Yes, either from excessive data errors or excessive hardware failures.
First you’ll see an A100 alarm for reset due to excessive data errors. If the problem continues, you’ll get an M5069 alarm when the device is locked out.
For excessive hardware failures, you’ll see an A101 alarm, followed by M5072 alarm for lockout.
In both cases, the system will ignore the port until you unplug the device and plug it back in.

The phone in question will say "System Lockout" on it. OR you can pull the error information while in database programming. Under Operations\Error Information. Save it to a folder, then view the .log files (text) It should show the errors DND ON mentioned.
