I have a CS-5200 at version 2.1.2.38 with a 256 CF. It’s been working fine since first installed in 2006.
I added a third LSM-4 and four new lines. Any calls placed via the new card drop after 5 to 10 seconds and the LSM resets. We swapped out the LSM, then the entire chassis, and the problem remains. The next step is to upgrade the software and put in a 1GB CF. The system is overdue for an upgrade anyway.
Just curious, anyone run into this before? I’m thinking that the problem is an anemic CF.

The only other thing I can think of would be the minimum revision level for the LSM-4 and it should be AE. The chances that both cards you tried are below that level are slim and none but I thought it might be worth mentioning.
Have you monitored Message Print while seating the card and while duplicating the problem?
Have you swapped the card with another one to see if the problem follows or remains?
Does the problem happen when calling in on those lines too or only when placing a call?
I do not disagree with you that the problem is probably the CF and the system is due for an upgrade.
Let us know what you find.

Both new LSM-4s are revision AR.
Message Print shows "Board 3 reset by IOC".
Swapped cards and lines - problem stays in slot 3.
Incoming calls are dropped as well as outbound.

Update – this appears to have been several defective LSM-4 cards, bad out of the box.
We replaced the CF and upgraded the system to version 5, and the problem remained. After trying yet another LSM, the problem disappeared.
We tried one of the LSMs in another system, and the same issue occurred. All cards were revision AR.

Thanks for reporting back DND ON. Those multiple bad out of box problems are very frustrating to troubleshoot.
BTW, I was unaware that you could upgrade the old CS5000 chassis to the new 5.0. I thought that version was only for HX5000. Good information to have.
Thanks again.
