I'm sure it's just operator error, but both our receptionists say that when they transfer calls to one extension, it quite often says "transfer failed" and they have to try 2-3 times before it goes through. I've called from my cellphone and watched them and every time they transfer me to that extension it works.
Their process: Answer call, press TRANSFER, dial extension number, speak to person, hang up, call should transfer.

More than likely it is user error.
Does the transferred to extensions ever have their phone forwarded offsite. There are certain circumstances where the transfer will fail if that is the case which might also explain the intermittent nature of the problem.

We have some phantom extensions that forward off site, but I don't think anyone else even knows it's possible.

I seem to recall having an issue like this when they were transfering to an IP phone and they where on DND.
I forget what corrected it, maybe I had to "zap" the phone.
Sorry it's not an answer. But if they are in DND maybe it helps to toubleshoot.
