You can't connect a proprietary phone to a pots line. It won't work.

Bell sonecor equipment makes a device called the "modem mate" that should enable you to connect your modem to any phone system. They list in the MCM catalog for $70.00. It's worth looking into anyway.

In order to run the SMDR, you will need a AF-355 adapter. Connect that to a DB-25 to DB-9 pin converter and you won't need to worry about the pin outs. The Af-355 adapter has an Rj-45 plug to run a cable from in to the SMDR outlet.

You can plug a general purpose adapter into the "other" jack on the bottom of the BIS 34 to connect the modem (or any single line device) to it. A 355A or 355AF adapter is the best way to connect to the smdr port.
