I have a Partner Rel 7 with a large VM card installed and about once every other week the passwords reset back to the default. Is the card going bad?

I quess you are talking about the PC card mail (12 box).
I had one do me that way a few years back and the only way I was able to correct was to go through and re-initalize all the boxes. And made sure the admin password wasn't the default 1234. I also after re-initalizing changed all the user passcodes to something different.
I suspected that someone may have just been calling in and screwing around with there mail. But not certain.

Yes, get into the habit of re-initializing both the small and large PCVM's when they are new before you do any programming. Just change the configuration to a different number of mailboxes. That will reset everything and make that problem go away. You can then change it back if you want to.

There is a know issue with new cards having some "residual" programming in them from the factory. It will cause mailbox assignments, auto attendant selector codes, and mailbox passwords to return to default after a power failure.
The Avaya remedy is to perform a Master Clear before doing ANY initial programming.
