We have 1 extension that the message waiting indicator does not work when this person receives a vm message. This only happens occassionally. We have a Definity G3 and an Octel VMX 200 Serenade voicemail system. Any ideas what could be causing this?

delete the mailbox and re add it back in. Then leave a test message. Did that work?

You could also try to busyout the port so you wouldn't have to delete and rebuild the box. I have an Avaya Definity G3si switch and a Definity voice mail 3.2 so you may want to find out if you have permissions in your switch to do this and what the sequence is. For my Definity I do the following:

busyout port xxxxxx
release port xxxxxx
