Trying to setup this 9040 handset for a merlin plus, it's working OK except that Line 1 is on the "B" on the first horizontal column, & lines 2,3,& 4 are on the 2nd column 1, 2, & 3. How can I get the 4 CO's to go onto the middle (horizontal) column on 1, 2, 3, & 4. ?

Check the 9040 button mapping table. From the phone, choose Menu, Configuration, Button Map (you must hit the double up-arrow a few times to see this option), then choose Merlin.

See page 80 (Adobe Acrobat page 88) in the MDW 9040 Wireless Pocket Phone Installation and Use guide for details on how this ultimately maps out.

You may also want to experiment with the Merlin 410/820 button map, which is illustrated on page 81 of the Pocket Phone guide mentioned above.
