did you assign the lines in auto select ?
I would assign ICM then the lines you want for outgoing I recently had a similar problem and that was the solution

I have set a System Password and Line Access Restriction to "Incoming Calls Only" on the TV room's extension, #22.
Um, take away the Incoming Only restriction.

No, the auto select goes straight to an Intercom so she can't get to an outside line.
With Line Access Restriction set to "No Restrictions", it allows unrestricted access to the line, and takes me back to the original problem of her being able to dial out. I've also tried the "No Access" option but the HOLD button and password has no effect when I select a line.

System Password does not override Line Access Restriction.
You will either have to build a restriction table or forget all that and use Station Lock.

How about if you set a system password in #403
Remove the Line Restrictions in #302
Set up Disallow List No. 1, entry 01, as "wild"
Assign Disallow List No. 1 to Ext. 22
Remove 911 from the Emergency List since your daughter is fond of dialing it.
You will go to the phone, press Hold, dial the password, then lift the handset and place your call.
If your daughter lifts the handset, or presses speakerphone, she will get dial tone, but if she dials anything, she will get disconnected.

Well, good. The advantage to doing that is you can now just assign other extensions to Disallow List #1 if she is able to get at them.
