That is Suttle's line of snap-in jack modules. Not much different than any other manufacturer's. I have not seen even a flush-mounted jack that can be used for an RJ31X. I know that Suttle used to make an SE635B68-50 that provided one 6P4C jack and one 8P8C jack with shorting bars, but I seriously doubt that you'll find any supply houses that still stock them.
