here is the smdr log as listed above to try:
11/05 09:10A 00:00:12 7007 1xxxxxx4364 T123 11/05 09:10A 00:00:00 T101 *** 754364 E
Looks like the PBX is responding to the inbound number, what does the 754364 mean? 4364 is the extension that was called, why is it showing 75 before it? Not sure what it means.
Anyone know what that indicates?

It looks like some kind of digit substitution going on on the trunks but you say the others are working correctly. It has to have something to do with the number of digits that are being repeated by the CO - unless you only have certain DID's going to certain channels on the PRI and the new DID's going to another group. I still vote for some kind of problem with the CO

The carrier is sending you more than 4 digits!! Isn't 754364 the last 6 digits of the phone number???????

Make sure you check your trunk service assignment for the correct number of digits to absorb or insert. If you are getting 4 digits make sure you have a 0 for number of digits to absorb. You can't leave it blank.

Also, use ccs trace to see exactly what the carrier is delivering.

Trunk service assignment and Class of service assignment are definitely in the right direction...
According to the SMDR logs all the incoming calls have the 75 pumped in front of them like this:
11/05 03:19P 00:01:33 T101 003 754302 4302 4302
Somehow it translates 754302 into 4302 and the call goes through.
My other did range works fine this way, just this new one not working.
Still learning!

OK, now it all makes sense. When they set this 3300 up they used system speed calls to forward the extensions.
So the provider sends 4 digits, the PBX takes it and adds 75 in front, then there is a system speed call that takes 75xxxx and forwards to xxxx.
Wow, that was brutal....
Thank you all for your assistance, the thing that did it for me was checking the trunk service assignment to see that the 75 was being pumped in.

The only reason they did that was, that there must be some conflicting DID's coming in that needed to be modified.
