Set up CO's 9, 10, 11 as DIL to x304, and DIL No Answer Dest to VM. I took the CO's out of all trunk and ring groups.
If x304 is idle, the lines ring in as expected and roll into VM if not answered. If x304 is off hook (icm or CO), the lines will not ring, instead the button turns solid red and the lines ring at x308!
This same system has CO's 7 & 8 DIL to x301, and it works just fine (second call comes in on CALL 2). So as a test I DIL'ed 9, 10, 11 to x301 and it worked fine. I then tried to DIL 9, 10, 11 to x308 and when off hook they go to 312 (similar to x304)!
All extensions can receive CALL 2 from another internal extension. All extensions are COS 1. All extensions are fixed CFWD NA to VM. No extensions are using station level CFWD.
Why can't some extensions receive CALL 2 from outside DIL? Why are they being "re-routed" to another extension?

Have you checked program 1027? This defines call forwarding types. 0=disabled, 1=fixed call fwrd with both ext ringing, 2=Fixed call fwrd when unanswered, 3=fixed call fwrd immediate, 4=fixed call fwrd when busy or unanswered.
And 1029 Fixed Call fwrd when busy.

Prog 1027=2 to VM: "All extensions are fixed CFWD NA to VM."
Prog 1029 is not available on 124i (according to the manual 384i lower than v3.04 only).

Prog 1003=> Take all stations out of Group. & Prog 0419 Item 12=> Enable off hook ring

Can see why 1003 would work. Was 308 just picking up? How about a call coverage key.
0419 would do it, s well as 0406:5
