OK i am able to reproduce the problem and have learned a little more...
I call someones extension from an outside line, they pick up, we talk or whatever, we hang up. Now this is the interesting part... When i ring thier extension it sometimes says 'call from 404'. It usualy says 'call from voicemail'. When it says 'call from 404' then after we hang up the ghost call will come into the operators ring group. I have since changed it so that the ghost call only comes into the receptionists extension.
So before the the person picks up the 'legit' call it says 404, then when the ghost call comes in it says 405. That must be a clue. I just cant figure it out.
does this make sense to anyone?

Have you looked in the system programming yet to see what these numbers are?

Extension 404 & 405 are probably analog voicemail ports. If I had to guess I would say the 4th & 5th ports (401, 402, 403, 404, 405 etc.) Find extensions 404 & 405 in programming and find out what is different about those extesions. Sounds like they are not a part of the voicemail group.
