Here's my problem. It only occurs about 30% of the time.
MICS 5.0 and a PRI. NAM 4.0.04C with 4 channels.
I have outbound transfer set up on a phone for customer initated transfer to a cell phone by pressing 7.
When it works this is how it goes: 1. Call rings in. 2. After 2 rings, FNA to the NAM. 3. At the desk phone I hear the V-mail xfer "da-duh" sound and see the caller ID. 4. The caller hears the greeting and presses "7". 5. The caller hears the "boop-boop" of being placed on hold. 6. The call rings to the cell phone.

Any ideas on why it sometimes fails? I was able to have all of the involved equipment in the room with me so I know the cell phone had service. Why is the v-mail xfer tone presented again at the desk phone when the call fails?

Traditional outbound transfer uses a 2nd line and conferences the two lines together.
The outbound transfer failed because A) the outbound transfer channel(s) were busy or B)there were no lines available in the line pool. Try increasing the number of available channels.
Better would be to set up the transfer as a Centrex type transfer using a hook flash as the 1st digit in the dial string, if the lines are equiped with the feature. It will always work as you are using the same channel and line as the one the call is already on. It also frees the line once transfer is completed.
