Yes, both lines (four wires) on the port are connected. The Auto Configuration syncs the clock and downloads the extension list, so the integration appears to be working.

All COs are in trunk group 1 ports 1-24 are assigned to service group 1 trunk groups 1-64 are N/A
I've checked trunk group settings and the corresponding timers. I tried making them shorter, longer, setting lines to different trunk groups. Nothing changes.
The only place I'm seeing for CID routing is in Service Settings > CallerID/DID/PIN Call Routing There are no entries in this section.
I put one in, routing my cell phone to my extension and the system tried to transfer it to the correct extension (according to the trace log.) My extension did not ring, and all I heard on my cell phone was ringing. SMDR again reads No-Answer.
After I removed it, it started AA service again.

brokeda had it right, but I think there might be more to it. I moved the VM to jack 27 (on 8EXT module #2), a jack we've never used, never configured an extension on, in fact, we just installed the module two days ago. The system picks up outside rings now. I moved it to jack 18 (on 8EXT module #1), doesn't pick up. I moved it to jack 26, on module #2, but we've programmed an extension there. Doesn't pick up.
What is happening to the old extension? Is there a setting I'm missing when I set [117]?

You should only need to set 117. Some ports get weird and don't work 100%. If it works, leave it alone.

And you don,t need to set ext group or hunting, setting a port as VM is all that is needed. setting a ext hunt group for voicemail is only when you are using a pc based voicemail that is connected to single line ext's and is using inband signaling its not for dpits the td/tda already knows that the voicemail ports are in a group.

**if your using extension ports on expansion cards for dpits connection to the tva200, make sure that when testing you have the grounding screws in the expansion card and that the system cover/door is closed. which completes the grounding. if the screws are not in and the door to the system is not shut then your card is not properly grounded and may cause weird problems with dpits communications
also make sure you have a good third prong ground or a seperate ground connection.
at panasonic tech support we useto recommend using ports 1-16 for dpits connection to a voicemail

on a dual cabinet system we recomended using ports 1-16 on the master or 33-40 on the slave. we found that ports above #40 had problems sometimes with dpits voicemail connections.
