Hello. Please help on the following problem.

Situation:
-Verified CO line has caller id. (directly connect to analog phone with caller id display)
-has TDA 50 with T7630 3 line display phone only showing "CO line number -> 601(ICD group)"
-phone is connected to one of DHLC4 port.

What are the program settings I should double check?

Do you have CID daughter board on the CO line card?
Is it turned on?

Do i need 5193 board or CID is supported by default in the base unit of TDA-50?

You will need the 5193 daughter board which connects to the LCOT4, and enables caller id for the 4 co lines on that card. You may need to go into programming, however by default the programming is enabled for caller id. Word is that Panasonic is soon to announce the TDA50G. It will have the caller id board installed at the factory for the first 4 co lines.

I have the CID card installed and turned on, but I'm still getting the same display as above "CO line number ->601(ICD Group). What now?

sure cid is coming in from CO??
check with slt or butt in.

You've verified that CID is enabled in your CO line properties? Also remember that CID is delivered after the 2nd ring. You're not answering too soon are you?
