We have a customer with a rack mount 128 7.2 with a T1 card in slot 2.
We have lost 3 T1 cards in the past 6 months. The system has surge protection on both the AC and T1 ckt. After the last bad card was replaced it would not work in slot 2.The system went completly down when the card was put in that slot.We moved it to slot 6 and no problems. We are replacing the cabinet today. We have never lost any other cards and have no trouble with the other T1's in the building.Has anyone had similar problems with either T1 cards or slot 2 in this type of cabinet? The telco has changed the facilities for this ckt and we have installed shielded cable and T1 ckt protection after the first lost card and we have lost 2 more.Any ideas???

To be honest with you, I've never had a T1 card go bad ever. It's rare when a Telrad fails like that. You are on the right track by replacing the cabinet. Take a good look at the cabinet and see it you can spot anything that may have caused it to short out the board.
SS may have run into this but I never have had it happen to any of our systems.

By rack mount you mean the Unit'e cabinet (14 slots)
I do remember a issue a few years back on the 128 in the 6 slot cabinet that the T1 shouldn't be installed in the 2 or 3 slot someting about highways to the expansion cabinets. I have to dig back for that Tech Memo. In the 400 if you have a sax or wax expansion card you have to move the T1 to slot 4 or above. The old 400 cabinet is a 15 slot cabinet.
You do have the correct firmware (chip) on the T1 card for the software level of the system???
