Thanks for the feedback, now im kind of confused by your posts, should the MOH be on violet/blue as you mentioned in your first post, or black/slate as mentioned in your second post?

It's violet/blue for the Mach 1 or Triad S It's slate/black for the XTSc
They are two completely different systems.
Sorry about the confusion. I tried to address the two of you with separate posts so it "wouldn't be confusing"!

Is the MOH Input different from the Background Music option? Have it punched down to the violet/blue and no Background music, that is how I was testing to see if it was working. I appreciate the help, as this is my first Vodavi.

No, the same input pair is used for MOH and BGM on a standard system. Dialing 6321 turns this music on at the phones and 6320 turns it off. 6322 would turn on a second source if you had it connected.
If you are not receiving BGM, I tend to think that the input transformer in the KSU for music has been blown during the troubleshooting process. Many times, when people can't get MOH to work, they jack up the volume of the music source to maximum levels. All that does after a few minutes is blow the input circuitry.
Could this be the case? There really is nothing else to do except connect the headphone output to the violet/blue pair. I will tell you that there is possibly a programming issue where a MOH source can be designated on a per-line basis, but I doubt that's your problem. Again, if background music isn't working, then it really has to be a failure in the input wiring or blown KSU electronics.
I'll be happy to assist you further, but at this point, I am at a dead-end. I would really like to hear from you when or if you are able to get this problem resolved because it's the first instance I have encountered with this problem.

Well I tried changing the source I was playing out through (was using a CD player)worked just fine with headphones plugged into it listening it. I also tried playing from the headphone jack output of a computer, however that didn't work either. When I plugged the headphone jack into my iPod it works just fine... strange...

Have you tried attaching your buttset in monitor mode to the end of the cable? Do you hear music?
You may try removing the red wire and just use the white and the shield. See what happens then.
