I'm 99% sure that if you connect a T1 card in a PBX to a smart jack that all you need really is an ethernet cable (568B). As long as you have pins 1-2/1-2 and 4-5/4-5 straight through you are good to go.
Correct??
Or is a smart jack considered a smart device, which you would need a cross-over T1 cable.

A smart jack is NOT a "smart" device. It's "smarter" then an RJ-48X jack, in that it can be accessed remotely. But it does NOT produce a T-1 signal. A multiplexor, a channel bank, a DS-1 Microwave radio - all these things produce a valid T-1 signal.
A jack or a dsx bay will not.

Straight-through should do it.
If you normally think in terms of DCE and DTE, then you're bound to get confused. The PBX's T1 card is technically the DTE (it derives timing from the CSU/DSU, which plays the DCE role). But in the case of the smartjack-to-T1-card connection, a straight-through cable is really just rejoining a deliberate break along the DCE path.
Why is there a "broken" link between the carrier and your CSU/DSU? Because there is a legal demarcation between the carrier's network and your equipment. Because of that, it behooves them to introduce a test point...one at the END of their network with gear that can respond to loopback requests and such. What? You thought they were gonna trust YOUR gear to do that?
An ethernet cable is overkill and by some accounts less preferred than a voice grade patch cord (cheapie silver satin junk will do). There are times when a T1 crossover cable is needed, but not here.
