I wonder if it might be a message notification. Just a shot in the dark here, but instead of a MWI being sent it could be Calling 401. I haven't played much with the ICE and it was about 3 years ago when I did so don't laugh at me too hard.

OK, the Tadiran tech did a cold start and replaced the firmware. The freaky issue with 401 has not reoccured yet however all calls from the PSTN to numbers (POTS) programmed to ring directly to extensions are all going to the auto attendant and to individual mailboxes. I have the "Predifined FWD" on a noans set to ring to 96:253 for each extension.
Does anyone happen to know if a UCD group can forward to it's own mailbox on a noans?

253 is the UCD group for voicemail, in your CO line programming what is each line programmed to ring day and night, is the system in night day mode or auto try switching pgm 63 and the password for that extension.

The issue with ext. 401 started again today at 9:03am as posted above on January 10, 2008 07:16 AM. Too many problems to address on this site. I have sent an SOS to Tadiran America. IF any of these issue every get resolved I will reply with resolutions. Maybe an IPX 500 is the answer at this point.

I would certainly talk the customer into an IPX500 or an IPXOffice. They are more advanced and solid systems.

It almost sounds like you have a voicemail issue. I dont know if I would be hasty and switch to an ipx500 although it is definatly a far better product. There really is no comparison, the ICE is an entry level ksu, the IPX is a pbx. I have installed several of both the ice and ipx this past year and found that both are very reliable. Only issues that I have found is a few out of box failures after that it has been smooth sailing.

I am going to go ahead and replace the expansion cabinet (CAB4) with a CAB 2. The main KSU is a CAB 1. I've been told by someone with extensive ICE experience to at least try this.

Just an update: I haven't done anything yet other than replace the firmware and it fixed nothing. All issues still exist. the current one I'm working on is 3 analog extensions in a UCD group are getting an ear piercing screetch when another call hits the UCD group (230). I've changed from "ALL Ring" to Linear just a few minutes ago, so I'll check tomorrow.

What voice mail do have in the cabinet? Could you maybe disable it for a test to see if it is cabinet or voicemail related? I've not heard of compatibilty problems between Cab1 and Cab4, but wouldn't be suprised.
