My curiousity just made me walk back and look at the ground setup for the cabinet. A 10 guage ground wire on the cabinet is joined up with about 3 other 12 guage ground wires. These grounds all then run on a single 10 guage wire up into the ceiling to who-knows-where.
I'll try a ground test on Monday.

Inter-tel called me this morning and I chatted with the tech about the echo issue. I mentioned all the troubleshooting steps I've taken so far.
Tomorrow he's going to come by with a SLC card and a LSC card and we are going to swap those two cards out one by one and see if the echo goes away.
If that doesn't work then I'll try the ground test.

Kaefer, go to trunk programming on the Axxess. System>Devices and Feature Codes>Trunks. Change the Hybrid Balance setting to 'Short' on all trunks. Test w/ that setting. If that doesn't help, make sure you set that back on all trunks.

Noooo. If you ever fire up VOIP sets w/ analog service, you need to adjust the hybrid balance setting to 'Short' to help alleviate echo. The 'Normal' setting will give you fits. I'm not sure this will fix the problem in this thread, but it's easy to change and probably worth trying.

I changed the setting from 'Normal' to 'Short' on the trunk these phones are attached to and the echo disappeared. Then I switched it back to 'Normal' while I was on the phone and the echo came back. The Hybrid Balance setting change was the fix! I owe you guys. Thanks again. -Kaefer Now I need to figure out what to do about voicemail and the upgrade to V9.0 for caller id on the SLC card.

Oh yeah, is there anything I need to know about this Hybrid Balance setting?
Should I expect that this setting may negatively affect all the rest of the phones we have, mainly Inter-tel digital desk phones?

I have never had it effect anything negatively. The hybrid balance setting really just tweaks the impedance setting for that incoming trunk. If test calls on the digital fones work fine and you can fax and modem call fine, then you should be in like Flynn...
