Second Error:
Error 0X80040119: CreateForm failed
I did a search and came up with a few things to try, disabling AV, removing files and re-install office 2003. We currently have 20 machine all with the same operating system / Office load and now three of them are getting these two errors.

Did you ever find a resolution for this problem?
What are you using for your email server?

We had a technician come in from ATI. He let us know that UM is not compatible with Office 2003 SP3. In order to get it to work, I had to uninstall office, then reinstall without SP3. Also avoid Microsoft update KB943649... it lists this as a possible cause of concern as well.

(This message is crossposted from another thread. Since so many people were having the problem, I thought I'd post it in the 2 threads related to this issue. If any mods take exception to this, please feel free to delete my posts. This fix is for Office 2003.)
Hail and hurrah! I have have found the culprit, and I have defeated him! His name.... is OUTEX.DLL! He lives in "C:\Program Files\Common Files\System\MSMAPI\1033"! His version is (at least) 11.0.8200.0! To defeat him, merely copy a previous version (I used 11.0.8000.0) over him, click either the fax or voicemail buttons, AND THEN REJOICE!
Apparently, my version (2.212) only needs this to be done once, I can copy the newer version back and it works fine.
For this, I humbly request two things (if they are possible):
1. A method of deploying the UM client besides manually.
2. A copy of the latest client (I have to *SCHEDULE* a time for my installer to contact me, usually over a week), which I can't find anywhere online.
All hail VMWare, the greatest troubleshooting tool known to mankind!

Moktarino, I must say that you ROCK!!!!!!!!!!!!!
I have been yelling and screaming at my Inter-Tel reps for months to get me a fix for this and they are either totally inept or are simply unwilling to to so.

They are unlikely inept or unwilling. Inter-Tel has made the decision to not support certain versions of Outlook. What that means is if you have a unique problem not readily fixable in the field, your local rep will have no factory technical/engineering support. That puts both them and you in a extremely poor position. While there may be work arounds to get the UM client working on unsupported versions, without factory support you are on your own when problems arise. As long as you can except that, great.
