After dialing code to cancel call forwarding, it still does the same thing. Dial ext 4626 then a 2 appears then dead silents.

I find that sometimes the UX5000 does strange things for no reason. Most of the time, that can be resolved by a system reboot. Have you tried that? Also, even though it's not likely the phone set - I would swap it out with a known working set just to rule that out.

I have reboot and changed the phone out. I also have changed the card out and unplugged ext at phone system. I get the busy like the is not connected when I do this so I do believe someone has type some code into phone to cause the problem. Anything else besides call forwarding that would give this problem??

Does that extension work properly for placing calls? Internal and external? Does the display on that extension appear normal? Check program 10-03 and make sure the port this phone is connected to is being recognized as a MLT in the terminal type.

It is a analog phone and she can place calls to internal and external numbers.

Check external call forwarding. Heck check ALL call forwarding in WebPro.

All call forwarding canceled at the handset. And nothing set in WebPro.

Sounds like the user may have inadvertently set Do Not Disturb option #2. If you go to the phone and dial 8470, that should cancel the DND setting.
