I have a customer who has 2 PRI on his OS7400. When calling out to any number that has a AA/IVR/Phone tree on it the call rings back to the receptionist phone. Please advise

Local or Long distance? Are LCR tables setup properly? Also check PRI switch type... NI2 is standard but you may not have a standard carrier.
I've never seen a call going out ring back into a PBX. Most likely something in LCR not completing the call out and sending call into Operator ring group.

Does it ring back to the receptionist phone at the same location? Does it disconnect the caller and make it ring back? Is the receptionist at the other locations?

I will check to see if LCR is enabled, but I don't think it is. They only have one location, so it is ringing back to the receptionist here. You can see that the PBX grabs a port off the PRI (7123) then it rings back to reception still showing the port it grabbed on the display.
