This morning when I came in to work; I have an error/alert message "Ext 6561 OFF Hook". Is that normal or is there anything wrong with it?

It's not normal. It was either someone who genuinely left their phone off the cradle, or it might be a defect in the keyset. Check the phone, reset by unplugging, clear the alarm and see if it happens again.
