Swapping in a new HD would probably be a freebie fix since I have a few extra HDs. Does the HD need to be formatted??
I think you are forgetting one important thing, the software. Even if you had a HD of the correct type you would have to find a way to format it for some proprietary OS then load the VS R5.0 software. Only problem is you have no way of getting it unless you work for Avaya.
The link DTMF gives has drives already loaded, just make sure you order the correct one and hope that that is the problem or this could turn into a costly experiment with money that would be better spent on a new mod.

hbiss is correct, you better be sure it's the hard drive and not the board or you will be wasting your money.

Or talk to the guys in the repair forum.
You could simply send your mod in and get fixed.

Don't want to step on any toes here, but from what you have told us about your problem I think all you need is a new hard drive in your mail VS r.5.
Would proably be less expensive and easier to get yours repaired.
I can remotely help you get it set back up.
But again this is my opinion.
I have replaced HD's in r.5 from serval years ago that are still up and working. For some reason the r.5's had a issue with HD's.
