vad60: There aren't any TTRs in any 408 module.
telephony121: Follow merlinman's advice and bolster your current TTR count; his first post makes complete sense. I suspect that outside calls to the MLM are handled slightly differently than they are with inside calls. Perhaps merlinman can comment on any differences, but that can be the only explanation for the one-sided failure.

My mistake. Maybe you can move you lines to 400 GS/LS-TTRs and see or that solves all problems?

TTR's are a shared resource in the system, and are used sequencially. Moving the lines to a different card is not going to make a difference. Follow ttech's adivce - get a Tip/Ring card and test the TTR's. I assume you have checked the error log??

Rotary, as in dial-pulse?!? For outbound calls? How does that affect incoming calls to the auto attendant?

I appreciate all of the input. I moved the lines to the 400 card already, the failure still exists (sequentially). I could probably insert an analog card and test (T&M), but at this point I've offered an upgrade to a Legend 7.0 and adding Merlin Messaging. The customer initially bought the system via ebayand thought they were saving some dough.It has never performed right, now it's just as much perception. I'm guessing (because of the failure rate)that their is a bad TTR in the 007MLM and probably a bad one in the 400 card.

Just a thought - if you set up remote access, you can test the ttr's remotely and won't need to install a t/r card. After the initial studder dial tone (unless you set up a barrier code), enter *04, the 2 digit ttr and you're ready to go.
