I have 5 instances of recently installed
(IP500 V2's REL 6.0 & 6.1 with 5410 sets)
having a light hum/whistle during inbound and outbound calls and this can only be heard at the customer/IP500 end of the call. Internal calls are fine. I thought the common factor was possibly the combo cards but have changed those out and still hear the noise. I have even replaced the control unit at two of the sites. I have tried the impedance test's and changed other line settings like gain adjustments, quiet line and hum filters. I have tried it with the system grounded/ungrounded. I have removed line protection. Reversed the polarity on the lines. All to no avail. The C/O lines sound fine on my test set ahead of the system.
First is anyone else having this problem on the IP 500 V2?
Is there a solution or does anyone have other things that I can try?

Have you ran this by your distributor jfreeman? They can open a trouble ticket with Avaya.
Just a thought.......
