Not familiar with this phone system - customer complaining that when calling an extension from one location to another he is getting constant busy - do not disturb - thought it might be a programming issue but not sure how to resolve - anyone know how to fix?

On every phone or just 1 .
If its only 1 swap the phone with a known good one line cord and all and see if it resolves

the phones on that system could be 20-23 years old.

Are any of the keys lit?? blf's.. you might have a key programmed for dnd and not labled or labled correctly that's turned on ..maybe even the lights out on it..try another phone there..just some basic suggestions...

Actually I didn't do anything - it was a trouble a friend was having so he ended up calling on a Toshiba tech - turns out they went onto the line that had the busy or DND and pressed the numbers 4 and 2 which cleared the busy - quite simple if you knew what to do!
