How do you enable digital ports 9-16 and what pins on the 66 block are used to terminate the phones?

they should already be enabled if you are carded for them , and X-209 - 216 , should start on the red/brown , brown red

I can see the extensions programmed in the system, however, the error I am getting is Disable by Fault: DO.

They are automatically there even if it is not carded for 16, is the system carded for 16 or 8 ??

I just took over this system, I guess I will have to go out on site and check out the card configuration. I know there are 2 cards to give me 6 CO trunks. Would that also give me the additional extension ports that I will need?

Yes, and if the GCDU is already in the system, all those digital ports should be active.

Make sure it is seated properly too, with the top and side pins... that could cause the "Disabled By fault" error. (maybe even do a reset, before going onsite)
