I have come accross this various times with different dial tone providers. What's going on is that compression on the PRI is turned up to high and messes up fax transmissions. To fix it I've had to do two things, 1- Talk to the dial tone provider and had them make adjustments on their end. 2- when making adjustment on their end doesn't work, they've had to give me the DIDs being used for faxing as analog lines off of the PRI. One more thing too now that I'm thinking about it, make sure they are providing the clocking or you are providing clocking. That can screw things up.

Another point to be wary of with faxes on extension ports is that most faxes are designed with a PSTN line in mind. the problem is a pstn line works at 48V whereas an extension port works at 24V. I have had some faxes that refuse to work on an extension port (HP multifunction centres most recently). I have also seen instances of the fax machine deteriorating with age in this respect.
If it is a clocking issue you usually get the fax through but there are slips and corruption in the image.

One other thing to consider is that the carrier is using voip or your customer is to dial out. Since you haven't changed anything speak to the carrier and find out if they are using voip. Faxes can be a bit temperamental on voip.
