Cant think of any great companies. I do hosted call center but that's a whole different ball of wax.
Problem with hosted PBX is people still try to run if through a cable modem.

I think there all great... so great that when I am pulling there stuff out and replacing it with an actual phone system im nice enough to box it up for them..

We do traditional and IP systems as well as Hosted. This thread says a lot and most true. You have to be very careful of your expectations. We do it and we do it well, but once you get over 6 phones in one site it is always best to look at traditonal systems with SIP trunks if you still want the low cost of lines. Hope that helps, not trying to be a salesman, but our Hosted works have hundreds of customer that will tell you the same. But be fearful of some companies out there, they are just in it for the quick buck

My company does some high-volume termination for call centers and almost all problems are related to the last-mile into the CPE. Once the signal gets back to the CO there are relatively few if any issues. So in my experience, the biggest problem with hosted/VoIP anything is the inadequate internet connections coming into the building. When your DSL or Cable Modem averages 2-3% packet loss that's a problem.
The other problem is fly by nights. All it takes to be an ITSP is a website and a $200 "server". Real ITSP's will insist on some sort of dedicated point to point or MPLS/CoS connection.

I think one of the biggest things that can kill a hosted solutions company is over selling to the expectations of the customer. Telling the customer they should have no quality issues with 50 people using 1 T-1 for connectivity. I used to love going to a potential customer and just ripping the hosted solution a new one!
