I'm all too familiar with this issue. It often costs less for IXC's to off-net their terminating traffic to 3rd party providers rather than use LEC end office or tandem trunks. You might run into a situation where tandem trunks are 5th on the route list and direct end office is 6th.
It all boils down to the "bottom line". Some of these providers do a good job, but others are plain terrible.

THANKS for posting this comment and the FCC link. We also continue to experience this problem with our clients, particularly the dead air drops on open calls. Problem is not limited to rural service areas. Appreciate the help!
