I have a strange issue with an extension on a DX-120. One particular extension is showing busy (lamp is lit) and all calls are going directly into their voicemail. There is no call forward set on their line, just Busy Dest and NoAns Dest. The phone works fine for outgoing calls. Even when the phone is physically unplugged from the DX120, that extension still shows as lit (busy) on all the other phones. It's on the DPM16 board.
The diagnostics on that port show "Exec. Display, Passed".
The symptom appeared a few months ago, lasted about a month, then reverted back to normal. Now it is back again and I'm at a loss.
Can anyone suggest additional troubleshooting steps.

I have had it several times, never found the root cause but a cold start and reload fixes it for a few months or sometimes,forever.

Do you have default numbering on the system? If not, I've seen that cause weird issues like this.
