I have 6 IP phones and one IP/SLA remotely from the switch working through the Internet. All works, well until...
I have expanded the ports open for UDP in the firewall twice, from a range of 6000-6500 to 6000-7039, and today to 6000-8000. Each time, because remote IP phones could no longer hear audio off the system.
So, will this go on forever or is there some upper number that is enough?
Total, the system has 66 IP phones and 2 analog phones on IP/SLA.

what system and what version? Sounds like you do not have your endpoints configured for NAT, one way or no audio is a tell-tall sign of NAT traversal failure or as you have already explored RTP port blockage.

The system is a 5400. The version is 2.3.6.73.
Are you saying System > Devices and Feature Codes > Endpoints > <endpointid> > IP Settings > NAT Address Type should be NAT instead of Auto?

Yes that is the setting; Auto should work, if it doesnt you can try setting it to NAT.
v2.x UDP 6004–7039 (RTP and RTCP) • Processor Module (PM-1) base port – 6004 • Processor Expansion Card (PEC-1) base port – 6604 Inter-Tel Protocol (ITP) 5566/TCP(ITP Call Control) 5567/UDP(ITP Call Control) Enable only if using ITP IP endpoints outside the firewall.

Well, setting the remote phones to NAT under System > Devices and Feature Codes > Endpoints > > IP Settings > NAT Address Type seems to have done the trick. The installing verdor' tech was out yesterday and called Inter-tel tech support. They did not believe this was nessary, but it works. So, I have set the UDP range to 6004-7039. We also moved the "remote" phones to System > IP-Related Information > 2 (Remote) > Endpoints from System > IP-Related Information > 1 (Local) > Endpoints
So, time will tell, but it looks good.
