admin uses barge in feature extensively. Admin is COS 2 All others COS 1 Getting a complaint that she can barge in on some but not others. Display says barge in failed. Can't see any diff in phones.

I have this same problem in a call center. I think I have narrowed it down to extensions that have been moved with set-relocate. Is it consistently the same extensions? My customer says it moves around. He resets the system and it's fine for a while too.
A while ago I had a customer who's employees figured out they could not be barged in on if they were on a conference call so they would make a conference call with their cell phone and the intended party to get around it. Shows the same error of Barge in failed too.

Reset system. Most barge in OK Two stations, sometimes yes and sometimes no ???
